ACP Journal Club. PCI using drug-eluting stents had higher mortality than CABG in diabetes and multivessel CAD.

چکیده
Commentary CABG and PCI techniques have been cornerstone therapies for patients with CAD (1). Central tenets of clinical decision-making include the notion that patients with an increased burden of CAD and worse left ventricular function have better clinical outcomes with more aggressive modes of treatment (2). Optimal medical therapy has improved over the years, as have clinical outcomes from PCI, with movement from balloon angioplasty to bare metal stents to DESs. CABG techniques have also evolved and now include widespread use of arterial conduits, better cardioplegia, and improved perioperative care.

Long-Term Mortality After Coronary Revascularization in Nondiabetic Patients With Multivessel Disease.
BACKGROUND In diabetic patients with multivessel coronary artery disease (CAD), the survival difference between coronary artery bypass graft (CABG) surgery and percutaneous coronary intervention (PCI) favors CABG. However, there are few data on the mortality difference between the 2 strategies in nondiabetic patients. OBJECTIVES This study performed a patient-level meta-analysis to compare th...
متن کاملQuality of life after PCI vs CABG among patients with diabetes and multivessel coronary artery disease: a randomized clinical trial.
IMPORTANCE The FREEDOM trial demonstrated that among patients with diabetes mellitus and multivessel coronary artery disease, coronary artery bypass graft (CABG) surgery resulted in lower rates of death and myocardial infarction but a higher risk of stroke when compared with percutaneous coronary intervention (PCI) using drug-eluting stents. Whether there are treatment differences in health sta...
متن کاملCoronary Artery Bypass Grafting vs. Drug-Eluting Stent Implantation for Multivessel Disease in Patients with Chronic Kidney Disease
BACKGROUND AND OBJECTIVES There is currently a limited amount of data that demonstrate the optimal revascularization strategy for chronic kidney disease (CKD) patients with multivessel coronary artery disease (CAD). We compared the long-term outcomes of percutaneous coronary intervention (PCI) with drug-eluting stents (DES) versus coronary artery bypass graft surgery (CABG) for multivessel CAD ...
متن کاملDiabetes and coronary revascularization.
CONTEXT Patients with diabetes mellitus account for approximately 25% of the nearly 1.5 million coronary revascularization procedures performed each year in the United States and experience worse outcomes compared with nondiabetic patients. OBJECTIVES To summarize the current state of evidence comparing the effectiveness and safety of coronary artery bypass graft (CABG) surgery and percutaneo...
متن کاملComparing coronary artery bypass grafting with drug-eluting stenting in patients with diabetes mellitus and multivessel coronary artery disease: a meta-analysis.
OBJECTIVES Although drug-eluting stents (DESs) reduce the rate of target vessel revascularization compared with bare-metal stents, the results of DESs for patients with diabetes and multivessel coronary artery disease (CAD) in the DES era are inconsistent. This meta-analysis was undertaken to assess the efficacy and safety of coronary artery bypass grafting (CABG) compared with drug-eluting ste...
